It was not until the 20th century that the cemetery and its grave monuments were moved from Kostheimer Hauptstraße to Mittlerer Sampelweg and considerably enlarged to an area of 39 hectares. As in Kastel, burials in the Kostheim cemetery also take place exclusively in underground graves, as the burial types were based on the customs of the Mainz diocese.

Memorial plaques from the First World War are attached to a wall of the mourning hall, which is connected to the entrance area in the south-eastern corner of the cemetery grounds. Right next to it is a field of honor for the fallen of the Second World War.
