Replacement driver's license in the event of loss or theft
If your name has changed, you do not need to have a new driving license issued. However, it may be useful to be in possession of an EU driving license with up-to-date information, especially when driving abroad.
If you wish to change the name on your driving license, a new card driving license will be issued to you on request.
If your driving license has been lost or destroyed (theft, illegibility or loss), you will need a new driving license unless you waive your right to drive.
If your driver's license is lost or stolen, you are required by law to have a new one issued. A temporary driver’s license will be issued for the duration of the processing period, which takes approximately four weeks. The new driver’s license can be mailed to you upon request. You may be required to submit a sworn statement, which is subject to a fee.
Processing time
The time it takes to process an application for a card-style driver’s license depends on the circumstances of each individual case. The new card-style driver’s license is produced centrally by Bundesdruckerei GmbH in Berlin. To receive your new driver’s license, you can request that it be mailed to you. Direct mailing is handled by Bundesdruckerei GmbH. Alternatively, you can pick up the completed card-style driver’s license in person or—upon presentation of a power of attorney—have a representative pick it up for you at the driver’s license office.
It is also possible to request express delivery of the driver’s license. The new driver’s license will then be available at the driver’s license office within a few days.

If you have changed your name:
Official proof of place and date of birth (e.g., ID card or passport), and, if applicable, a certificate of residence,
previous driver’s license
a biometric photograph in accordance with the Passport Regulation (35 x 45 mm, full-face view, portrait orientation). Information and examples can be found in the photo sample chart (Bundesdruckerei).
In the case of a name change due to marriage: additionally, a certified copy of the marriage certificate, provided the name has not yet been changed on the ID card or passport,
In the case of a name change due to divorce: additionally, a certificate of name change from the civil registry office, provided the name has not yet been changed on the national ID card or passport.

A biometric photo can be taken on site (a self-service terminal for digital photo capture is available. Costs 6,00 Euro)
Fees
The fee is based on the scale of fees for road traffic measures (GebOSt).

Remark
If a driver’s license is lost, the applicant may be required, at the request of the administrative authority, to submit a sworn statement regarding the whereabouts of the license.
In order to use the services of the Driver’s License Office, you must be registered with your primary residence in Wiesbaden.
