If your name has changed, you do not need to have a new driver’s license issued. However, especially when driving abroad, it may be advisable to have an EU card-style driver’s license with up-to-date information.
If you wish to have your name changed on your driver’s license, a new driver’s license will be issued to you upon request.
Official proof of place and date of birth (e.g., ID card or passport), and, if applicable, a certificate of residence,

In the case of a name change due to marriage: additionally, a certified copy of the marriage certificate, provided the name has not yet been changed on the ID card or passport,
In the case of a name change due to divorce: additionally, a certificate of name change from the civil registry office, provided the name has not yet been changed on the national ID card or passport.

If necessary, submission of a sworn statement regarding the whereabouts of the driver’s license

Fees
A fee is charged for the issue or exchange of a driving license. Information on the amount of fees and costs can be obtained from the competent driving license authority, which determines these in accordance with the German Road Traffic Fee Regulations (GebOSt). The amount of the fees depends on the administrative costs. The costs for sending the driving license, its express delivery or other expenses are not included in the fees, but are also to be borne by the applicant.
