The following are required for an extension to categories D1, D1E, D and DE
Valid identity document
valid electronic residence permit for foreign passports
current biometric photo
first aid certificate
medical certificate (not older than one year)
ophthalmologist's certificate (not older than two years)
Performance certificate from an occupational physician (not older than one year with the following contents: Resilience, orientation ability, concentration ability, attention performance, reaction ability). Evidence can be provided e.g. by a company or occupational medical report from an officially recognized assessment body for driving fitness).
Name of the training driving school
Current driving license
Transcript of index card (list of driving license data if it was not issued in Wiesbaden). This copy of the index card can be obtained from the authority that issued the current driving license. If you are already in possession of a card driving license, you do not need to obtain a copy of the card.
Fees
The fee is based on the scale of fees for road traffic measures (GebOSt).
